Mail or fax the completed form (address and fax number above).  The online 'quiz,' below, can be completed at a library, school, or home. 


Truancy-related Quiz   (students)

If you're a student
directed to complete an online quiz (and assignments), follow instructions below:

1:  Set up an account: click on: Student Registration. It is important to keep your user ID and
        password; write them down.  You'll need the same User ID when doing your written
        assignment
(short answer questions, below).

2:
 'Click' on: "Identifying Information" and provide that information (you will need your user ID
        and password)
.

3:  'Click' on:  'QUIZ (True/False & Multiple Choice & Short Answer).' (you will need your user ID and password).

    It is important to 'plan.'  Some courts and schools require you to show that you completed the quiz (usually, Lena Pope staff report directly to the court, instead).  You can print your results to take to school or court.  If you don't have a printer when you take the quiz you can login later ('gradebook') and print the results (you will need your user ID and password).  Libraries usually offer printing at about ten cents per page. Think about your answers; you'll have only one chance to take the quiz.
